There are multiple ways to complete the forms.
You only need to follow the instructions of one of the four methods below.

1. Click on the fields you would like to complete and type in your information, then click on the "Submit the form" button at the bottom of the page.

3. Right click on the link below, select Save Target As and save the PDF file to your hard drive where you can complete it, print it, and fax it to (954) 727-0024.
4. Print the form and fill it in by hand (with black ink) and fax it to (954) 727-0024.
